Talking about death and dying can be difficult especially for care staff who are already under significant pressure. Not everyone feels comfortable having these conversations.
I can support care home staff by having open conversations with residents to establish an Advance Care Plan, which can then be used by the home to facilitate a person’s wishes when the need is there. Having these conversations can enable people to remain in their place of residence rather than being taken into hospital when wishes are unclear.
I also run Advance Care Planning Workshops for family members who have Lasting Power of Attorney for a loved one to help them identify how they can make end of life decisions and an end of life plan for a loved one.
