Advance Care Planning (not advanced care planning – it’s not that complicated!) is a way of recording your wishes so that those around you know what you would want to happen in the event that you lose capacity to make decisions for yourself.
I am certified by Living Well Dying Well to provide advice on Advance Care Planning. This can include recording your wishes and preferences – a document that simply outlines your likes and dislikes and what you would choose, or writing an Advance Decision to Refuse Treatment, which is a legally binding document to ensure you are not given treatment to prolong your life if you would not want this.
My one to one Advance Care Planning service includes an initial discussion to explore what is important to you and to help you think about those important things that will matter to you in the event you are not able to make or communicate decisions. We can consider everything from where you would like to be and how you would like to be treated all the way through to your likes and dislikes – if you don’t enjoy something now, would you want it forced on you when you are unable to refuse?
This discussion has no time limit, we work at your pace.
Advance Care Planning – Statement of Wishes and Preferences.
You can then either use this first discussion to write your own Wishes and Preferences document (a template is provided) or I can take the information we discuss, write it up and return it to you, where during a second session we can make any adjustments. You will then receive your statement by email.
Advance Decision to Refuse Treatment
An Advance Decision to Refuse Treatment (ADRT) is a legally binding document in which you can specify which treatments you would not want to receive under which circumstances should you lose the power to make or communicate these choices. Following our initial meeting we would then have a second discussion to define your choices clearly. I would then write up your decision with the necessary terms to make it legal and return five paper copies and one digital copy to you.
